For covered uninsured/self-pay estimate situations, scheduling 3–9 business days ahead generally requires the estimate within 1 business day of scheduling; scheduling 10 or more business days ahead generally allows 3 business days. A request without scheduling generally requires a response within 3 business days. Check applicability and current federal guidance.

## Record the trigger date

CMS explains different timelines for scheduling and requests. A request can trigger the estimate process even when no appointment is booked. Do not confuse a good faith estimate of charges with a good faith clinical exam; they solve different problems.

## Make the deadline visible to the team

As an operational control, save the request or scheduling timestamp, appointment date, uninsured/self-pay status and delivery deadline together. Track when the written estimate was delivered and preserve its version. State rules or other obligations may add requirements.

## Practical checklist

1. Ask whether the patient has or intends to use insurance for the service.
2. Record the scheduling or estimate-request date.
3. Calculate the applicable business-day deadline.
4. Deliver the written estimate and retain the exact version sent.

## Worked example

A patient requests expected charges before choosing an appointment. The request should enter the estimate queue immediately, rather than waiting for a booking. Use the request date to calculate the response deadline and retain proof of delivery.

## Mistakes to avoid

- Treating a verbal price as the complete written estimate.
- Waiting until check-in for a visit booked weeks earlier.
- Confusing the financial estimate with a clinical exam.
