Comparing marketing partners? See how many new patients a monthly fee needs to break even →

For med spa owners, plastic surgeons and practice marketing leads. Updated October 22, 2026 · Prepared by Prospyr. Prospyr makes practice software, not marketing services, and none of the agencies below is a Prospyr product or partner. This list compares nine AI SEO agencies that say, on their own websites, that they optimize medical aesthetic or healthcare practices for AI search: ChatGPT, Google AI Overviews, Gemini, Perplexity and similar tools. Each summary reflects what the agency publishes on its own site, checked October 2026. AI search is new and changing fast, and no agency can guarantee what an AI tool will say, so confirm scope, reporting and contract terms in writing before you sign.

What does an AI SEO agency do?

An AI SEO agency works to get your practice named and cited when patients ask AI tools questions like "who is the best Botox injector near me" or "how much does a tummy tuck cost," on top of ranking you in regular Google results. The work goes by several names: generative engine optimization (GEO), answer engine optimization (AEO), AI search optimization or LLM optimization. In practice it is mostly the same set of tasks: clear, answer-first treatment and procedure pages; FAQ and structured data (schema) markup; consistent practice facts across your website, Google Business Profile and directories; reviews, press and other third-party mentions that AI tools draw on; making sure AI crawlers can read your site; and testing real patient prompts to see whether your practice appears.

Most of that is good SEO done thoroughly, so the questions that separate agencies are narrower: does the firm know aesthetic medicine, what does it actually measure for AI visibility, and what does it cost.

How we chose this list

We searched for agencies with a dedicated AI search, GEO or AEO service and kept those that (1) describe that service on their own website, not only in a blog post, (2) name med spas, plastic surgery or aesthetic practices as a market, and (3) say something checkable about what the work includes, such as the platforms they test, the markup they add or how they report. Then we looked for published prices and contract terms.

We left out agencies whose main product is practice software, CRM, AI receptionists, booking or review tools, because those compete with Prospyr, and agencies that mention AI search only in passing. The order is our editorial view, not a measure of results. For broader SEO and marketing options, see our lists of med spa SEO companies and med spa marketing agencies.

Questions to ask any AI SEO agency

  1. Which patient questions will you test, on which AI tools, and how often? Agree on a written prompt list for your city and treatments before you start.
  2. What is your baseline? Ask for a before snapshot of where your practice appears today, so later reports mean something.
  3. What is AI-specific and what is regular SEO? Most of the work overlaps. Ask which deliverables are new and what they cost.
  4. How will this tie to booked patients? Mentions are not consultations. Ask how AI referral traffic and calls will be tracked to bookings.
  5. Who approves treatment claims? Your provider or medical director should approve anything about results and safety. Our advertising compliance reviewers list covers outside review.
  6. What do you guarantee, exactly? No one controls what an AI tool says. Read any guarantee for how success is measured.
  7. Who owns the content, schema and accounts if we part ways? Get it in writing.

Where Prospyr fits

AI search can send a patient to your website, but the booking still happens in your practice software, and that is where you find out whether AI visibility is paying off. Prospyr's lead capture and lead management record where each inquiry came from, online scheduling books the visit on one patient record, and analytics show which sources turn into treatments. Prospyr's AI agent can also answer patient questions around the clock, trained on your own services and policies. Ask any agency to tag AI referral traffic so it shows up in your reports, and book a practice demo to see how.

Frequently asked questions

What is AI SEO?

AI SEO is the work of getting a business named and cited in answers from AI tools such as ChatGPT, Google AI Overviews, Gemini and Perplexity, as well as in regular search results. For a med spa, it usually means clear treatment pages and FAQs, structured data, consistent listings, reviews and outside mentions, and testing real patient questions to see whether the practice appears.

What is the difference between GEO, AEO and SEO?

SEO aims to rank pages in search results. AEO (answer engine optimization) shapes content so search features and AI tools can lift a direct answer from it. GEO (generative engine optimization) aims to make a practice a source that AI tools trust enough to name when they write an answer. Agencies use the terms differently, and most AEO and GEO work builds on regular SEO.

How much does an AI SEO agency cost for a med spa?

Few agencies publish prices. Of those that do, Med Spa SEO Agency lists plans at $950 and $1,499 a month, KKV Labs lists $1,000 a month with a 3-month minimum, and Sagapixel states it plans around budgets from $1,500 to $20,000+ a month, all as of October 2026. Most others quote after a call.

Can an agency guarantee that ChatGPT will recommend my med spa?

No agency controls what an AI tool says, and answers change from one question and one day to the next. Some agencies offer guarantees tied to a written list of test questions; read exactly how success is measured. Fuel Online, for example, states it does not promise specific rankings or citations.

Do I need a separate AI SEO agency if I already have an SEO company?

Often not. Ask your current SEO company what it does for AI search, which prompts it tests and how it reports. Several firms on our med spa SEO companies list now include GEO in their SEO work. A separate specialist makes sense if your current firm has no answer.

Want your company considered?

We review this list quarterly. Agencies that offer AI search, GEO or AEO services to med spas and aesthetic practices can send their website and what they offer practices to info@prospyrmed.com. Inclusion is editorial and not paid.
