For clinic owners, practice managers and HR leads. Updated October 2, 2026 · Prepared by Prospyr. Prospyr makes practice software, not background checks, and none of the companies below is a Prospyr product. Each summary reflects what the company publishes on its own website, checked October 2026. Packages, turnaround times and prices change, so confirm terms in writing before you order.

What does a healthcare background check include?

A healthcare background check confirms a new hire's identity and screens their criminal history, then adds checks a general employer usually skips: professional license verification, federal and state exclusion and sanctions lists, and often drug testing and immunization records. A screening company, known legally as a consumer reporting agency, runs these searches with the candidate's written permission. For a clinic, the useful differences between providers are which healthcare searches come standard, whether they keep monitoring after hire, and whether prices are published or quoted.

How we chose this list

We loaded each company's own website and kept screeners that publish healthcare-specific searches, not just general criminal checks. Few screening firms name med spas or aesthetic practices, so this is a list of healthcare screeners, and we note which ones are built for hospitals. We looked at four things a practice manager can confirm before signing:

  • Healthcare searches. Does it list license verification and exclusion screening (OIG, SAM, state Medicaid lists)?
  • Monitoring after hire. Does it re-screen or alert you when something changes?
  • Published pricing or a small-account path. Can a clinic that hires six people a year buy without a volume contract?
  • Accreditation. Does it state accreditation by the Professional Background Screening Association (PBSA)?

The order is our editorial view, not a measure of results.

1. Checkr

Checkr reports that more than 140,000 businesses use its platform. It is the most transparent on price of any company here, which matters for a small clinic hiring a few people a year.

What stands out:

  • Published packages (as of October 2026). Basic at $29.99 a report, Essential at $59.99 and Complete at $94.99, with enterprise pricing by quote. It states there are no commitments or contracts.
  • Healthcare add-ons. FACIS healthcare sanctions searches from $6 to $15.50 a check depending on level, professional license verification at $12 a check, and drug testing from $37 a test onsite or $60 at a collection site.
  • Small-business option. Checkr also owns GoodHire, which sells the same three tiers at the same starting prices for smaller employers.

Right for: a single-location med spa or wellness clinic that wants to order checks as needed and see the price first. Ask: which package plus add-ons covers a nurse injector, and how FACIS levels differ.

2. Universal Background Screening

Universal Background Screening, based in Phoenix, is PBSA-accredited and serves companies of all sizes. Its healthcare package lists multi-jurisdiction criminal searches, verification with state licensing boards, NPI and specialty credentials, OIG, GSA and Medicare/Medicaid exclusion searches, employment verification, continuous re-screening, and optional drug and alcohol testing. It names clinics and outpatient care centers among the healthcare organizations it supports.

What stands out:

  • Re-screening built in. It lists continuous re-screening as part of its healthcare offering.
  • Clinics named. Unlike several larger screeners, it lists outpatient clinics directly.

Right for: clinics that want healthcare checks and ongoing monitoring from one vendor. Ask: the price per hire for your package, and whether there is a minimum annual volume.

3. PreCheck (Cisive)

PreCheck is the healthcare division of Cisive, based in Texas. It lists background checks, drug and occupational health testing, license monitoring, immunization tracking, electronic I-9, employment verification, and exclusion and sanction screening. It reports 30+ years in healthcare and names hospitals, clinics and physician practices as customers. Cisive states PBSA accreditation.

Right for: clinic groups that want a screener that works only on healthcare hiring and can track licenses and immunizations after hire. Ask: whether it takes on single-location practices, and what the minimum contract is.

4. Verified Credentials

Verified Credentials offers criminal searches, ID verification, professional credential checks, drug testing and driving records, plus healthcare searches including FACIS reports, OIG sanctions, GSA excluded parties and abuse registries. It states it is a PBSA-accredited founding member, runs 100% US-based operations, and completes 84% of searches in under one day.

Right for: practices that want healthcare-specific searches with a quote built for their size. Ask: the package price for a provider versus a front desk hire.

5. Certiphi Screening

Certiphi Screening works only with healthcare organizations and reports 5,500+ healthcare customers since its founding in 1989. It lists pre- and post-employment screening, license tracking and verification, exclusion screening, electronic fingerprinting through Fieldprint's 2,100+ collection sites, immunization tracking, drug screening and E-Verify. It states PBSA certification.

Right for: larger groups, or practices in states that require fingerprint checks for certain roles. Its examples are mostly hospitals and staffing firms. Ask: whether it serves a practice your size and what fingerprinting costs.

6. Accurate Background

Accurate Background, headquartered in Irvine, California and founded in 1997, reports 16,000+ customers and 60+ million searches a year. It lists healthcare as an industry and offers criminal checks, sanctions screening, drug testing, I-9/E-Verify and identity verification. It states accreditation through the Background Screening Credentialing Council.

Right for: clinic groups hiring across several states. Ask: which healthcare searches its standard package includes, and whether license verification is extra.

7. First Advantage

First Advantage sells a healthcare product called HEAL (Healthcare Exclusions, Actions, and Licensure) covering license verification, federal and state exclusion searches and OIG and GSA debarments. It also offers drug testing, vaccination verification, criminal checks and ongoing criminal monitoring, drawing on 1,500+ sources.

Right for: large or fast-growing groups; its listed markets are acute care, post-acute care, staffing and life sciences. Ask: whether it works with outpatient practices directly.

Questions to ask any background check provider

  1. Which healthcare searches are in the base price? License verification and exclusion checks are often add-ons. Price the full package for a nurse injector and for a front desk hire.
  2. Do you monitor after hire? A check on day one says nothing about a license suspended in month eight.
  3. Is there a minimum volume or annual contract? Small clinics should look for per-report pricing.
  4. How do you handle FCRA paperwork? Ask whether the platform sends the disclosure, collects authorization and generates adverse action notices.
  5. How long do results take, and what slows them down? County court searches and international records take longer.
  6. Are you PBSA-accredited? Accreditation means an outside reviewer has audited its compliance practices.

The FTC lists the steps employers must follow under the Fair Credit Reporting Act: tell the candidate in writing, in a stand-alone format, that you may use the report; get written permission; before acting on the report, give them a copy of it and "A Summary of Your Rights Under the Fair Credit Reporting Act"; then send an adverse action notice if you decide not to hire. State and city laws can add more, so check with your employment attorney.

Where Prospyr fits

A background check comes before a new hire gets a login; your practice software is where their access starts. Once a new provider clears screening and credentialing, give them a role in your system that matches their license, so they can only chart and book what they are allowed to do. If you're reviewing practice software too, book a practice demo.

Frequently asked questions

Is a background check the same as credentialing?

No. A background check covers identity, criminal history and employment history. Credentialing confirms professional qualifications such as license, DEA registration, education and board certification. Clinics hiring providers need both; see our list of credentialing services for clinics and our post on common credentialing mistakes.

How much does a background check cost for a clinic?

As of October 2026, Checkr publishes packages from $29.99 to $94.99 a report, plus $12 for a license verification and $6 or more for a healthcare sanctions search. Most other healthcare screeners quote by package and volume. Budget for drug testing separately if you require it.

Do I need to check exclusion lists for med spa staff?

If any provider bills Medicare or Medicaid, yes. The HHS Office of Inspector General states that excluded people "can receive no payment from Federal health care programs for any items or services they furnish, order, or prescribe." For a fully cash-pay clinic it is still a cheap check that catches serious history, and most healthcare packages include it.

Do I need permission to run a background check?

Yes. Under the Fair Credit Reporting Act, as summarized by the FTC, you must tell the candidate in a stand-alone written notice and get their written permission before ordering a report from a screening company.
