For prospective med spa owners. Updated October 2, 2026 · Prepared by Prospyr. Prospyr makes practice software, not franchises, and none of the brands below is a Prospyr product or partner. Each summary reflects what the franchisor publishes on its own franchise website, checked October 2026. Fees, investment ranges and unit counts change with every new Franchise Disclosure Document, so treat these figures as a starting point and confirm everything in the current FDD and in writing before you sign.

What does buying a med spa franchise get you?

A med spa franchise gives you a brand, a treatment menu, a build-out plan, training and a support team in exchange for an initial franchise fee and an ongoing royalty on sales. Published total startup costs for the brands below run from $79,000 for a small studio format to more than $1.5 million for a full-size location. Most franchisors say you do not need a medical background, because treatments are delivered under a medical director or physician. What you give up is control: the franchisor typically sets your services, suppliers, pricing approach and often the software you run.

How we chose this list

We looked for brands that sell med spa franchises in the US and publish a working franchise website, then checked four things a prospective owner can verify before a discovery call:

  • Published economics. Does the franchisor post its franchise fee, total investment and royalty, or make you ask?
  • Operating history. How many locations does it report, and how long has the brand run its own practices?
  • Clinical model. Does it explain who supervises treatments and whether owners need a medical license?
  • Menu fit. Is the service mix mostly aesthetics, or does it lean on hormones, weight loss and IV therapy, which carry more state rules?

We excluded franchise brands whose website no longer shows an active franchise offering when we checked. The order is our editorial view, not a measure of results or franchisee profitability. Your state, budget and experience may point you somewhere else.

1. VIO Med Spa

VIO Med Spa is a physician-guided med spa franchise headquartered in Medina, Ohio, offering injectables, laser, wellness and facial treatments, including HydraFacial. It states it has offered franchises since 2019 and reports 65 locations across 20 states.

What stands out:

  • Published 2026 FDD figures. A $50K franchise fee, a total initial investment of $642,722 to $1,109,954, and a 6% royalty.
  • Clear financial bar. It lists $500K in liquid capital and a $1.5M net worth.
  • No industry experience required. Its franchise site says some owners come from health or medical work and others do not.
  • Memberships. It describes membership plans as part of its revenue model.

Right for: well-capitalized buyers, including multi-unit operators, who want the brand with the most locations on this list. Ask: what share of the 65 locations are open versus in development, the brand fund percentage, and which technology and suppliers are required under the FDD.

2. dermani MEDSPA

dermani MEDSPA is based in Windermere, Florida and offers Botox, Juvederm, laser hair removal and aesthetic skin rejuvenation. Its franchise site describes the brand as a top 25 provider for Allergan in the US and a top 5 provider for SkinPen microneedling.

What stands out:

  • 2026 FDD figures. A $55,000 initial franchise fee and a total initial investment of $491,792 to $906,189 for a single location.
  • Financial bar. $250,000 liquid and a minimum $1,000,000 net worth.
  • Term. Ten years, with two five-year successor agreement options.
  • No medical background required. It states treatments run under the license of a medical doctor.

Right for: buyers who want a narrower aesthetics menu without hormone or IV programs. Ask: the royalty and marketing fund rates, which are not on the franchise FAQ page, and how many locations are open today.

3. 4Ever Young

4Ever Young Anti-Aging Solutions, based in Boca Raton, Florida, combines aesthetics (neurotoxins, fillers, Morpheus8, IPL, laser hair removal) with hormone therapy, medical weight loss including GLP-1 programs, and IV vitamin infusions, sold through a membership model. It reports 133 units open or in development.

It publishes a $60,000 franchise fee, a 7% royalty, $400,000 liquidity and a $1,000,000 minimum net worth. The total investment differs between its own pages: $521,650 to $754,900 on its process page and $522,650 to $807,400 on its main franchise page.

Right for: buyers who want aesthetics and wellness under one membership and are ready to manage the extra rules that come with hormones and compounded or GLP-1 medications. Ask: which total investment range is current in Item 7 of the FDD, and how many of the 133 units are open.

4. Skinovatio

Skinovatio franchises a full-size med spa with 18+ treatments, including Botox, dermal fillers, laser hair removal, microneedling, Hydrafacials, Emsculpt Neo body sculpting, and tirzepatide and semaglutide weight loss programs. It publishes a $50,000 franchise fee, a total investment of $695,400 to $1,560,050, a 6% royalty and a marketing fee of up to 1.5% of gross sales. It states prior aesthetics experience is not required and that it provides proprietary systems for scheduling, customer management and financial tracking.

Right for: buyers who want body sculpting and weight loss alongside injectables, with a larger budget. Ask: what the high end of the investment range pays for, whether its scheduling and patient records systems are required, and how many units are open.

5. Beauty Refined

Beauty Refined offers two formats: a Studio of 150 to 300 square feet and a Premium location of 1,500 to 3,000 square feet. Its menu covers neurotoxins, fillers, lasers, microneedling, facials, peels, IV injections, medical weight loss and peptide therapies. It reports three locations in Colorado and Texas and a 2026 FDD.

It publishes franchise fees of $30,000 (Studio) or $60,000 (Premium), a total investment of $79,000 to $880,000 depending on model and market, and a 6% royalty plus a 2% brand support fee. It says a Studio can open in about two months and a Premium location in 9 to 18 months, with training in Parker, Colorado.

Right for: buyers who want to test the market with a small footprint before a full build-out. Ask: how the medical director is arranged for a Studio and what a Studio can legally offer in your state.

6. Healthy Solutions Medspa

Healthy Solutions Medspa runs two practices in Ivyland and Furlong, Pennsylvania and opened its franchise to physicians in 2026. Its menu covers medical weight loss, injectables, hormone and peptide therapy, body contouring, lasers, skincare, IV therapy and NAD+. It publishes a total estimated investment of $825,908 to $1,354,500, $250,000+ in liquid capital and an $825,000+ net worth, and lists 35 states where franchises are available.

Right for: physicians who want to own and lead a med spa and wellness practice under a brand. Ask: the franchise fee and royalty, and whether non-physicians can own.

7. Alexis Lauren

Alexis Lauren operates med spas in Bay Harbor Islands, Coral Gables and Winter Park, Florida, offering Botox and Dysport, fillers, Sculptra, PRP, laser skin treatments, laser hair removal, microneedling and peels. Its franchise page lists a $50K franchise fee, a 6% royalty and $500K in liquid capital. It does not publish a total investment range.

Right for: buyers who want an early position in a smaller system. Ask: the Item 7 total investment, how many franchised units are open, and which states it is registered to sell in.

8. MedSpa 810

MedSpa 810 is franchised by Phoenix Franchise Brands of Livonia, Michigan, which also franchises pet care, insulation and flooring brands. The med spa menu covers Botox and dermal fillers, clinical skincare, laser hair removal and non-invasive body contouring. It states no prior experience is needed. It does not publish fees or investment figures; you request them through a form.

Right for: buyers comfortable with a franchisor that runs several unrelated service brands. Ask: how many MedSpa 810 locations are open, and how much of the support team works only on the med spa brand.

9. Sapphira Privé

Sapphira Privé lists locations in Miami, San Antonio, Houston and Orlando, plus cities in Spain and Portugal. It offers laser hair removal, weight loss programs, injectables, laser rejuvenation and hormone replacement therapy. It states no industry experience is required. It does not publish fees or investment figures.

Right for: investors who want a brand with a mostly European footprint. Ask: how many US units are franchised versus company-owned, and how the US support team is staffed.

Questions to ask any med spa franchise

  1. Have I read the full FDD? Under the FTC Franchise Rule, the franchisor must give you its Franchise Disclosure Document at least 14 calendar days before you sign or pay. Read all 23 items with a franchise attorney, especially Item 7 (initial investment), Item 19 (financial performance, if any) and Item 20 (openings, transfers and closures).
  2. Who can legally own the practice in my state? Many states limit who can own a medical practice, so franchises often use a management company and a physician-owned entity. Ask how the structure works where you live, and check your state's rules in our med spa licensing guide.
  3. What software and suppliers are required? Items 8 and 11 disclose required purchases and computer systems. Ask what the booking, charting and payment systems cost each month and whether you can change them.
  4. What happens to my patient data if I leave? Ask who owns the patient records and list, and what the agreement says at the end of the term.
  5. What do current owners say? Call franchisees from the Item 20 list yourself, including some who left the system.
  6. What is the real all-in monthly fee? Add royalty, brand fund, technology fees and any required vendor programs.
  7. What does the medical director arrangement cost? Ask who recruits and pays the physician and what happens if they leave.

Where Prospyr fits

Most franchise agreements require the brand's chosen booking, charting and payment software, so a franchise owner usually cannot pick their own. Prospyr is built for independent and multi-location owners who choose their own systems, so the trade-off is worth weighing next to the franchise fee. If you go independent, see how Prospyr works for med spas and memberships, or book a practice demo.

Frequently asked questions

How much does a med spa franchise cost?

Published total startup costs for the brands in this list range from $79,000 for Beauty Refined's Studio format to $1,560,050 at the high end of Skinovatio's range, as of October 2026. For full-size formats, the published low ends start at $491,792 (dermani). Initial franchise fees run $30,000 to $60,000, plus ongoing royalties of 6% to 7% where published.

Do I need to be a doctor or nurse to own a med spa franchise?

Usually not. VIO, dermani, 4Ever Young, Skinovatio, Beauty Refined and MedSpa 810 all state that owners do not need a medical or industry background. Treatments are delivered under a medical director, and your state's ownership rules decide how the business must be structured. Healthy Solutions Medspa is the exception here: it is built for physician owners.

What is a Franchise Disclosure Document?

It is the disclosure the FTC Franchise Rule requires a franchisor to hand you at least 14 calendar days before you sign or pay. It has 23 items covering fees, initial investment, required suppliers, litigation, financial statements and lists of current and former franchisees. It is the only reliable source for the numbers in this article.

Is a franchise better than opening an independent med spa?

It depends on what you value. A franchise gives you a tested playbook, brand and supplier pricing, but costs a fee, ongoing royalties and control over your menu and systems. An independent practice keeps more margin and flexibility but you build every system yourself, often with a consultant. If you plan to open more than one location, compare the franchise's royalty over ten years against the cost of building your own.
