Printable face chart template: views and layout

The default printout is a front view and both profiles on one letter-size page, with a header, legend, product and lot table, notes and a signature line. Other presets give front with both obliques, front only at a larger size, or all five views.

  • Front: forehead, brows, eyes, nose, lips, chin, jaw and neck.
  • Oblique (three-quarter): useful for cheek, jawline and brow contour.
  • Profile: nose, lips, chin, jaw angle and neck.

Right views show the patient's right side and left views the left. Mirrored left views keep the same landmarks, so the two sides compare directly.

Injection mapping: how to use the chart

  1. Choose the views you need and fill in the patient, date and provider.
  2. Pick a symbol: neurotoxin, filler, other product or note. Rename the symbols if you like.
  3. Tap or click the face to place a mark. Tap a mark again to remove it, or use Undo.
  4. Print or save as PDF. Fill in product, lot and expiration in the table, and sign.

The marked-site counts in the side panel match the legend on the printout. Counts are sites, not units or volumes.

Botox face chart muscle regions: the key

With regions switched on, shaded areas carry these numbers. They name common muscle groups of the face and neck that are discussed as targets in the anatomy literature on botulinum toxin; see the source below.

The shaded areas are drawn to a generic face. Real anatomy varies between patients and between the two sides, so the provider's assessment of the patient in front of them always governs.

From paper face charts to the chart in your EMR

A paper chart is fine for one visit. It is harder to compare across a year of visits. Prospyr's charting keeps the treatment record, products used and before-and-after photos together in the patient chart, and the aesthetic EMR page shows how that fits the rest of the practice. Frequently asked questions

What is a face chart?

A face chart is a diagram of the face used to document where a treatment was given. Injectors mark it at the visit, so the next provider can see exactly what was done and where. It is also called an injection mapping chart or injection diagram.

What is a Botox face chart used for?

It records the treated areas for each visit alongside the product, lot number and the provider's notes. A chart from the last visit helps a returning patient get a consistent treatment, and a dated record helps if a question comes up later. This template adds symbols for neurotoxin, filler, other products and notes.

Which views does the template include?

Front, right and left oblique (three-quarter) and right and left profile. Pick a preset or switch each view on or off. The left views are mirror images of the right ones, so the sides match, and the chart labels say which side of the patient is shown.

What do the numbered muscle regions mean?

They label the surface areas where common facial muscle groups sit, such as the frontalis in the forehead and the masseter over the jaw angle. They are orientation landmarks and are approximate. They do not show where to inject, how deep or how much, and they are optional.

Does the chart tell me where or how much to inject?

No. It records what the provider decides and does. The marks carry no units or volumes, and the product and area fields at the bottom of the printout are blank for the provider to complete from the visit.

Can I save the marked chart?

Use Print and choose Save as PDF, then attach the file to the patient's chart. The page does not store or send anything, so closing it clears the marks. Do not rely on the page as the record.

Sources and scope

A documentation aid. The drawings and shaded regions are generic and approximate; they do not show where, how deep or how much to treat, and they recommend no product or dose. The treating provider decides and records the treatment.
