Hair Transplant Cost Calculator: Grafts and Price Per Graft
Hair transplant cost is the number of grafts times the price per graft, plus any fixed fees. To estimate the grafts, the published PRECISE method scores one point per 30 cm² of bald area and plans 1,500 grafts per point. An 80 cm² area scores 2.6 points, or 3,900 grafts. How many grafts do I need? The PRECISE method
The PRECISE scale (Castro and colleagues, 2024) turns the size of the area to be restored into a point score, then into a graft count. It was written for surgeons planning an operation, and it is the only published conversion from area to graft count we found that is open access.
- Measure the completely bald area (cm²) and the thinning area (cm²). Thinning counts in proportion to how miniaturized it is: fully above 70%, by percentage from 30 to 70%, not at all below 30%.
- Relative bald area = bald area + thinning area × that share.
- Points = relative bald area ÷ 30, in 0.1 steps (every 3 cm² is 0.1 point, rounded down), plus 0.1, 0.2 or 0.3 for each receded temple.
- Grafts = points × 1,500 (50 follicular units per cm² over 30 cm²).
Worked example (patient A in the paper): 60 cm² bald plus 40 cm² thinning at 50% gives 60 + 40 × 0.5 = 80 cm². 80 ÷ 3 = 26.7, rounded down to 26 steps, so 2.6 points. 2.6 × 1,500 = 3,900 grafts. The authors call 1,500 per point guidance that varies with the density the surgeon wants.
Hair transplant cost: grafts times price per graft
Hair graft cost is simple multiplication once the count is known. The part that is hard to find is a trustworthy price per graft. ISHRS's own census does not publish one. It reports an overall average fee used to size the market and says that figure "should not be construed as a typical price" for a procedure. That is why this calculator asks you to enter the price from your quote or fee schedule, and fills in example values only so a result shows on the first screen.
Worked example: 3,900 grafts at a $4 to $8 range is $15,600 to $31,200. If the plan is split into sessions of 2,347 grafts, that is 2 sessions, and a $500 fixed fee per session adds $1,000.
Hair graft calculator: what sessions look like in practice
The 2025 ISHRS Practice Census (covering 2024, from member surveys) gives the closest published picture of session size:
- Average grafts per session: 2,347 for a first procedure and 1,637 for a later one.
- Average grafts in a typical case: 2,262 (FUE) and 2,100 (FUT).
- Members reported 1.5 surgical procedures per patient on average to reach the desired result.

Frequently asked questions
How much does a hair transplant cost?
There is no single published price per graft. The cost is the number of grafts times the price per graft your practice or surgeon quotes, plus any fixed fees. The International Society of Hair Restoration Surgery (ISHRS) states that the average fee in its census should not be read as a typical price, because fees vary with graft count and other factors. Use the calculator with your own quote.
How is hair graft cost calculated?
Multiply the number of grafts by the price per graft, then add facility, anesthesia or other fees if they are billed separately. For 2,400 grafts at $5 per graft with a $500 fixed fee for one session, the total is 2,400 × $5 + $500 = $12,500. Many practices quote a range or a per-graft price that changes with the size of the session.
How many grafts do I need?
It depends on the size of the area to be restored and the density the surgeon plans, not on the Norwood stage alone. The PRECISE method scores one point per 30 cm² of bald area (counted in 0.1 steps) and plans 1,500 follicular units per point, which is 50 per cm². The surgeon makes the real plan after examining the scalp and donor area.
How many grafts are in a typical hair transplant session?
ISHRS members reported an average of 2,347 grafts for a first session and 1,637 for a later session in the 2025 Practice Census, which covered 2024. In a typical case, members reported averages of 2,262 grafts for FUE and 2,100 for FUT. These are member averages, not targets for any one patient.
Is there a maximum number of grafts a person can have?
The supply of donor hair is limited. A 2017 review in the Indian Dermatology Online Journal cites approximately a maximum of 6,000 follicular units from a safe donor area at the best donor density. The same paper notes that this can be inadequate for advanced Norwood stages VI and VII, which is why body hair is sometimes discussed as an additional source.
Does the Norwood stage tell me how many grafts I need?
No. Two patients with the same Norwood class can have different head sizes and bald areas. The PRECISE paper shows two Norwood III vertex patients needing 3,900 and 6,150 grafts by area, and a Norwood V and a Norwood VI patient needing about 9,000 and 9,300. Those are simulated head templates, so treat them as illustrations of the spread.
Can a practice use this calculator to quote patients?
A practice can use it as arithmetic for a first conversation: pick the price per graft from your own fee schedule, set the graft count, and read the total and margin.
