Patient Satisfaction Survey Template for Aesthetic Practices

A patient satisfaction survey for an aesthetic practice asks about booking and reception, the consultation and treatment, results and aftercare, the practice itself, and the overall experience, ending with a 0 to 10 recommend question you can score as a Net Promoter Score. Choose from the 24 questions below, add your practice name, then copy the survey into your survey tool or print it. Score the recommend question at the bottom.

How to run the survey

  1. Choose your questions. Start with the overall section, then add the sections you want to act on this quarter.
  2. Send it the same way to every patient. Use the same timing and channel for everyone, so the results compare from month to month.
  3. Read every written answer. The open questions tell you why a score moved. Assign each complaint to a person to follow up.
  4. Score the recommend question monthly or quarterly with the calculator above, and compare against your own last result.

Survey answers tied to a named patient and their treatment are patient information. Keep them with the same care as the chart, and ask your privacy officer whether your survey tool needs a business associate agreement before treatment details go into it.

Frequently asked questions

What questions should a med spa patient survey ask?

Ask about each step the patient went through: how easy booking was, whether they were seen on time, whether the provider listened and explained what to expect, whether they felt pressure to buy, whether aftercare was clear, how satisfied they are with results so far, and how likely they are to recommend you and come back. End with two open questions: what went well and one thing to do better.

How many questions should the survey have?

As few as answer what you need to know. Every question you add is one more reason for a patient to stop partway. Pick the sections that match a decision you are actually trying to make, and remove the rest.

How is Net Promoter Score calculated?

Ask how likely the patient is to recommend you on a 0 to 10 scale. Answers of 9 or 10 are promoters, 7 or 8 are passives and 0 to 6 are detractors. The score is the percent who are promoters minus the percent who are detractors, so it runs from −100 to 100.

When should I send the survey?

Ask about booking, reception and the visit soon after the appointment, while it is fresh. Ask about results once enough time has passed for the patient to see them, which depends on the treatment; the results question includes a “too early to tell” answer for that reason.

Can I send only happy patients to leave a review?

Be careful. The FTC's 2024 rule on consumer reviews bars incentives conditioned on a review's sentiment, and bars presenting the reviews on your own site as all or most of the reviews when negative ones were suppressed. Ask every patient the same way, and have your attorney review any review-request process.

Does this page show average satisfaction scores for med spas?

No. We do not have survey data to publish, so this page gives you the questions and the scoring math, not benchmarks. The best comparison is your own score from one quarter to the next.
