After your Sculptra treatment

Swelling, bruising and tenderness are common after Sculptra. Massage is part of the aftercare plan. Follow these steps unless your provider tells you otherwise.

The first 24 hours

  • Within the first 24 hours, apply ice or an ice pack wrapped in cloth to reduce swelling. Do not put ice directly on skin.
  • Skip hard exercise, extensive sun, UV lamps, indoor tanning beds and extreme temperatures for the first 24 hours.
  • Makeup can go on a few hours after treatment if there are no open wounds or bleeding.
  • Most patients can return to normal activities right away.

The first week

  • Bleeding, tenderness, discomfort, redness, bruising and swelling are expected. They usually go away within a few hours to a few days, and can last 14 days or longer.
  • Tell us if you have had a laser treatment, peel or hair removal recently. Recent skin therapies can increase redness, swelling, heat or pain.
  • Small lumps or bumps (nodules) can appear. They may be visible or only felt under the skin and usually go away on their own or after treatment.

What to avoid

  • Hard exercise, extensive sun, UV lamps, tanning beds and extreme temperatures in the first 24 hours.
  • Putting ice straight on your skin.

Call us if

  • Get medical help right away for unusual pain, vision changes, skin that looks white near the injection, or signs of a stroke (sudden trouble speaking, numbness or weakness in the face, arms or legs, trouble walking, a drooping face, severe headache, dizziness or confusion).
  • Symptoms that are getting worse or lasting longer than expected.
  • Lumps or bumps you can see or feel under the skin.

The Sculptra massage instruction: 5 minutes, 5 times a day, for 5 days

The FDA-approved patient guide for Sculptra tells patients to massage the treated area for 5 minutes, 5 times per day, for 5 days after treatment. Many practices call this the 5-5-5 rule. The AAD describes poly-L-lactic acid as the one filler that requires gentle massage, for about 5 minutes several times a day for 1 to 2 weeks, and says the dermatologist will tell the patient if they need it.

The two sources do not match on duration, so the template uses the FDA labeling for the default and puts a blank right beside it for your own technique and timing. If your protocol uses a different schedule, change the line. Nothing on this page is a recommendation to deviate from the label.

Nodules and Sculptra: why the call-us list names them

The Sculptra patient guide says lumps and bumps (nodules) are a reported side effect. They can be visible or only felt under the skin, and the risk can increase if large volumes are injected at the same place or deep wrinkles at the corner of the mouth are treated. They usually go away on their own or after treatment, though surgery can sometimes be needed.

A handout that names nodules sets the right expectation and gives the patient a reason to call you instead of massaging harder on their own.
